Support patient follow-up: Ensure records are updated, review test results with providers, and communicate with patients as needed.
Coordinate care: Manage patient medication prior authorizations, and assist with outgoing referrals and scheduling.
Perform administrative tasks: Support scheduling, patient check-in/out, insurance verification, and other administrative needs when needed.
Collaborate with the care team: Work closely with both clinical and front-desk staff to maintain smooth operations.
Maintain exam rooms: Ensure supplies are stocked, and rooms are ready for patient care.
Provide compassionate care: Adhere to knownwell&#39;s mission of providing inclusive, non-judgmental care to all patients, regardless of size, background, or health status.
Virtual care: May support virtual patient care needs as part of our hybrid care model, including remote check-in and clinician support when needed.</p>
<p>Requirements:
High school diploma or equivalent; Medical Assistant certification preferred.
At least 1-2 years of experience in a healthcare setting, preferably in primary care, specialty care, metabolic health, or weight management.
Experience with prior authorizations and referrals is a strong plus.
Strong organizational skills and attention to detail.
Excellent communication and interpersonal skills.
Familiarity with electronic medical records (EMR) systems (Athena experience preferred).
Ability to manage multiple tasks efficiently in a fast-paced environment.

      <Title>PACU Registered Nurse</Title>
      <Description><![CDATA[<p>As a PACU Registered Nurse, you will play a critical role in the immediate post-operative care of surgical patients. Your responsibilities will include managing airway support, closely monitoring neurological and cardiac status, and responding promptly to any complications or side effects.</p>
<p>You will provide initial wound care to the patients&#39; surgical site and ensure patient stability during recovery. You will collaborate with the surgical and anesthesia team. This role includes on-call availability during select non-business hours which will be discussed in the interview.</p>
<p>To deliver care and create a therapeutic environment for patients and families whose needs range from basic to complex by utilizing the nursing process. To carry out functions in a manner consistent with Performance Improvement and in accordance with hospital policies and procedures. To demonstrate clinical excellence through proficient practice which enhances and enriches the practice environment.</p>
<p>Proficiency is evidenced by adeptness, facility, aptitude, ease and skill in managing complexity and uncertainty.</p>
<p>Responsibilities:</p>
<ul>
<li>Assess patients&#39; physical and psychosocial status based on interview, examination and review of clinical data.</li>
<li>Assess potential for crisis situations and intervene before crisis situation develops with adeptness, facility, aptitude, ease and skill in managing complexity and uncertainty.</li>
<li>Reassess patient status at established time frames and as indicated by the patient&#39;s condition.</li>
<li>Based on assessment, analyzes relevant patient data and identifies nursing diagnoses/patient care problems.</li>
<li>Communicates patient status and assessed needs to members of multidisciplinary team.</li>
<li>Develops an individualized plan of care, including developmental and cultural needs, in collaboration with the patient, family and interdisciplinary team.</li>
<li>Reviews and updates plan of care as needed.</li>
<li>Acts as a resource to others in developing and implementing plan of care.</li>
<li>Uses the nursing process as a basis for developing, implementing and evaluating nursing interventions.</li>
<li>Provides patient-centered care with sensitivity and respect for the wealth of human experience.</li>
<li>Performs technical skills as needed by specific clinical unit safely and competently.</li>
<li>Coordinates and implements the medical diagnostic and therapeutic orders as directed.</li>
<li>Anticipates and manages complications of disease progression and invasive procedures.</li>
<li>Documents plan of care, assessments and interventions and patient outcomes in an electronic medical record thoroughly, accurately, and according to CMC Professional Nursing Practice Standards and ANA Scope and Standards of Practice.</li>
<li>Reports status, including response to care, and changes in status as appropriate.</li>
<li>Ensures calm, supportive and therapeutic environment for patients and their families, serving as an advocate when appropriate.</li>
<li>Adheres to all safety, organizational and ethical standards according to ANA Code of Ethics.</li>
<li>Implements elements of core measures in conjunction with Performance Improvement.</li>
<li>Identifies current trends which could impact patient outcomes.</li>
<li>Identifies system resources effectively and allocates them appropriately at the point of care delivery.</li>
<li>Manages a specific complex clinical procedure common to patients in area of practice, and encourages colleagues to seek her/his guidance in managing the specific procedure/technique throughout the year.</li>
<li>Employs critical thinking skills to solve problems and achieve patient outcomes</li>
<li>Identifies &amp; communicates needs for innovative nursing practices.</li>
<li>Identifies &amp; communicates need for new or revised policies, procedures, guidelines, etc.</li>
<li>Identifies research as a resource in clinical practice.</li>
<li>Stays current with new resources/knowledge/evidence</li>
<li>Serves as a resource/trainer for colleagues regarding competencies, policies and procedures, and clinical practice related to patient population served.</li>
<li>Utilizes the change process to improve patient care, the work environment, and patient and staff satisfaction.</li>
</ul>
<p>Minimum Qualifications:</p>
<ul>
<li>Licensure/Certification: Current registration in the State of NH as a Registered Nurse.</li>
<li>Basic Life Support (BLS) certification required.</li>
<li>Education: Graduate of an accredited school of nursing</li>
<li>BSN strongly preferred.</li>
<li>Specialty certification strongly preferred.</li>
<li>Experience: Minimum of 9 months of nursing experience.</li>
