Responsibilities:

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities and other care settings – 75-90% local travel
• Assess and evaluate member's needs and requirements in order to establish a member-specific care plan
• Ensure members are receiving services in the least restrictive setting in order to achieve and/or maintain optimal well-being
• Plan and implement interventions to meet those needs
• Coordinate services, and monitor and evaluate the case management plan against the member's personal goals
• Guide members/families towards resources appropriate for their care

Requirements:

• Must live in Collier or Charlotte county in Florida
• Bachelor's degree in health and human services field
• 2+ years of related experience
• Exceptional verbal/written skills
• Use of a variety of electronic information applications/software programs
• Valid state driver's license, reliable vehicle, and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits

Preferred Qualifications:

• Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
• Prior experience with Medicare & Medicaid recipients
• Experience with health promotion, coaching and wellness
• Knowledge of community health and social service agencies and additional community resources
• Bilingual Spanish/English

Scheduled Weekly Hours: 40 Pay Range: $53,700 - $72,600 per year
