YubHub-native raw fields carry `x-` prefix.","jobs":[{"@context":"https://schema.org","@type":"JobPosting","identifier":{"@type":"PropertyValue","name":"YubHub","value":"job_2351e1b3-ca5"},"title":"Analyst, Case Management Field","description":"<p>We&#39;re building a world of health around every individual , shaping a more connected, convenient, and compassionate health experience.</p>\n<p>This Analyst, Case Management Field position is with Aetna&#39;s Long Term Services &amp; Supports (LTSS) team and is a field-based position out of the Macon County area of Illinois.</p>\n<p><strong>Job Responsibilities</strong> As an Analyst, Case Management Field, you will facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support, and education for members through the use of care management tools and resources.</p>\n<p>Key responsibilities include:</p>\n<ul>\n<li>Conducting comprehensive evaluations of referred members&#39; needs and eligibility</li>\n<li>Recommending approaches to case resolution and/or meeting needs</li>\n<li>Coordinating and implementing assigned care plan activities and monitoring care plan progress</li>\n<li>Identifying high-risk factors and service needs that may impact member outcomes and care planning components</li>\n<li>Utilizing negotiation skills to secure appropriate options and services necessary to meet the member&#39;s benefits and/or healthcare needs</li>\n<li>Providing coaching, information, and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices</li>\n</ul>\n<p><strong>Requirements</strong></p>\n<ul>\n<li>Must reside in the state of Illinois in Macon County Illinois area</li>\n<li>Valid Illinois Driver&#39;s license</li>\n<li>Willing and able to travel up to 75% of their time to meet with members face-to-face</li>\n<li>Reliable Transportation required, eligible for mileage reimbursement as per company policy</li>\n<li>Minimum of two (2) years of case management experience</li>\n<li>Microsoft Office and electronic health record experience</li>\n</ul>\n<p><strong>Preferred Qualifications</strong></p>\n<ul>\n<li>Case management and discharge planning experience preferred</li>\n<li>Managed Care experience preferred</li>\n<li>Microsoft Office experience preferred</li>\n</ul>\n<p><strong>Education</strong></p>\n<ul>\n<li>Individual with a bachelor&#39;s degree or a non-licensed individual with a master&#39;s degree, with either degree being in a human-services field (including, but not limited to sociology, special education, rehabilitation counseling)</li>\n</ul>\n<p><strong>Salary and Benefits</strong> The typical pay range for this role is: $21.10 - $44.99 per hour.
