Responsibilities:

• Mentor and support Field Nurses and Care Coaches to ensure compliance with the DHS contract
• Identify learning needs and provide clear expectations, orientation, and ongoing learning to build capacity of IDT staff
• Assess and coach staff on IDT primary competencies and performance standards
• Review reports, assess processes, and review documentation
• Assess utilization and quality of care management for each member served
• Participate in cross-departmental collaboration to ensure quality and effectiveness of provider services
• Educate community partners
• Participate in hiring, orienting, and training associates
• Facilitate in-person unit meetings and attend quality visits

Requirements:

• Bachelor's degree in human services or a related field, or associate degree in nursing
• 1 year of leadership experience in human services
• 3 years of experience working in the Family Care program

Preferred Qualifications:

• 5 years of experience working with individuals with intellectual/developmental disabilities, physical disabilities, or frail elders
• Reside within 1 hour of Eau Claire
• Master's degree in public administration, social work, or a related field

• Salary range: $78,400 - $107,800 per year

You will report to the local Field Marketing Specialist (FMS) and be responsible for driving the brand image and product understanding on your campus and in the region.

• Building and executing a tailored and innovative campus plan to engage students and bring the brand to life
