Oak House supports up to six adults on the autism spectrum with complex needs.</p>\n<p>As a Support Worker, you will provide high-quality, person-centred support to help individuals live with dignity, independence, choice, and purpose. Your responsibilities will include:</p>\n<ul>\n<li>Providing person-centred personal care and daily living support</li>\n<li>Promoting choice, independence, and involvement</li>\n<li>Supporting individuals to achieve their personal goals and outcomes</li>\n<li>Encouraging participation in activities and community life</li>\n<li>Building positive relationships with individuals, families, and colleagues</li>\n<li>Maintaining accurate care records, plans, and daily notes</li>\n<li>Acting as a key worker to support individuals in setting and reviewing goals</li>\n<li>Maintaining safety, confidentiality, and professional standards</li>\n<li>Contributing ideas to continuously improve the service</li>\n</ul>\n<p>We are looking for someone with a caring and respectful approach to supporting others, able to work flexibly and think innovatively. Strong communication and relationship-building skills are essential.
