YubHub-native raw fields carry `x-` prefix.","jobs":[{"@context":"https://schema.org","@type":"JobPosting","identifier":{"@type":"PropertyValue","name":"YubHub","value":"job_5dc06451-884"},"title":"Paramedic for PCN including Home Visiting","description":"<p>The Coco PCN Home Visiting Paramedic is an exciting opportunity for an experienced and motivated Paramedic to make a meaningful difference to patients across the local community.</p>\n<p>Working across patients homes and GP practices, you will provide high-quality, compassionate and patient-centred care, using your clinical expertise to assess, diagnose, treat and support patients with a wide range of health needs.</p>\n<p>Key Responsibilities:</p>\n<ul>\n<li>Undertake comprehensive clinical assessments, examinations, diagnosis and treatment.</li>\n<li>Provide high-quality home visits to patients unable to attend the practice.</li>\n<li>Develop and review individualised care and treatment plans, including appropriate investigations and follow-up.</li>\n<li>Recognise and appropriately manage acutely unwell or deteriorating patients, escalating care when required.</li>\n<li>Work collaboratively with GPs and the wider multidisciplinary team to provide coordinated care.</li>\n<li>Support patients and families through clear communication, education and advice.</li>\n<li>Make appropriate referrals and onward care arrangements.</li>\n<li>Maintain accurate and timely clinical records.</li>\n<li>Participate in professional development, training and quality improvement.</li>\n<li>Work within all relevant clinical, safeguarding, confidentiality, health and safety and professional standards.</li>\n</ul>\n<p>The role combines home visits and practice-based clinics, assessing, diagnosing, treating and appropriately referring patients to provide safe, effective and timely care.</p>\n<p>Working alongside GPs and the wider multidisciplinary team, you will support urgent and routine care, improve access to primary care, reduce avoidable hospital admissions and improve patient outcomes.</p>\n<p>You will be expected to demonstrate strong clinical decision-making, work autonomously within your scope of practice, contribute to service development and deliver evidence-based, patient-centred care.</p>\n<p>Essential Qualifications and Experience:</p>\n<ul>\n<li>Registered Paramedic with the Health and Care Professions Council (HCPC).</li>\n<li>Educated to degree/diploma level in Paramedicine or an equivalent qualification.</li>\n<li>Holds a current Advanced Life Support (ALS) certificate.</li>\n<li>Has successfully completed the two-year Consolidation of Learning period as a newly qualified paramedic.</li>\n<li>Has a minimum of three years experience working at Band 6 or equivalent level as a paramedic.</li>\n<li>Holds, or is working towards, a postgraduate qualification in a relevant field, such as Advanced Paramedic Practice, Advanced Clinical Practice or Healthcare Management.</li>\n</ul>\n<p>Desirable Qualifications and Experience:</p>\n<ul>\n<li>Experience in primary care, community healthcare or home visiting.</li>\n<li>Experience of clinical audit and quality improvement.</li>\n<li>Working knowledge of EMIS.</li>\n<li>Additional community healthcare qualifications.</li>\n<li>Experience of working within or contributing to multidisciplinary teams.</li>\n</ul>\n<p>Benefits:</p>\n<ul>\n<li>Competitive salary depending on experience</li>\n<li>Permanent full-time position</li>\n</ul>\n<p style=\"margin-top:24px;font-size:13px;color:#666;\">Job Data by <a href=\"https://yubhub.co\">YubHub</a>.</p>","url":"https://www.jobs.nhs.uk/candidate/jobadvert/A0463-26-0010","directApply":true,"hiringOrganization":{"@type":"Organization","name":"Crouch Oak Family Practice","sameAs":"https://www.jobs.nhs.uk","logo":"https://logos.yubhub.co/jobs.nhs.uk.png"},"x-apply-url":"https://www.jobs.nhs.uk/candidate/jobadvert/A0463-26-0010","x-work-arrangement":null,"x-experience-level":"senior","x-job-type":"full-time","x-salary-range":"Depending on experience","x-skills-required":["Advanced Life Support (ALS)","Paramedicine","Clinical assessment","Diagnosis","Treatment","Communication","Teamwork"],"x-skills-preferred":["Primary care","Community healthcare","Home visiting","EMIS","Phlebotomy"],"x-benefits":[],"x-licences":["Driving licence","HCPC registration","DBS check"],"x-visa-sponsorship":"unknown","x-visa-sponsorship-text":null,"x-work-authorisation":"unknown","x-work-authorisation-text":null,"datePosted":"2026-10-26T10:45:50.414Z","jobLocation":{"@type":"Place","address":{"@type":"PostalAddress","addressLocality":"Addlestone"}},"employmentType":"FULL_TIME","occupationalCategory":"Healthcare","industry":"Healthcare","skills":"Advanced Life Support (ALS), Paramedicine, Clinical assessment, Diagnosis, Treatment, Communication, Teamwork, Primary care, Community healthcare, Home visiting, EMIS, Phlebotomy"}]}
